"hand up"/"hands up" can sometimes help with foreskin pull
Running the image through upscaling/highresfix can produce a 'double foreskin' effect. Removing the trigger words for the second pass can sometimes help with this.

Sampler: Euler a, 28 steps
CFG: 4 - 7
Clip skip: 2
